Position Summary:

 HF Connecting Health Nurse Practitioner-Led Clinic’s (the “Clinic”) provides culturally competent primary health care services with a focus on mental health through collaborative services, education, advocacy and research. Working closely with other administrative staff and interdisciplinary health professionals, the Medical Secretary will be responsible for client service at the front desk and implementing best practices throughout the clinic.

 

Medical Secretary is also responsible for the office coordination and facility management of the clinic.

 

Major Responsibilities:

  1. Front Desk Client Service
    1. Perform reception and reception-related duties in a positive and helpful manner when responding to routine inquiries from telephone calls, visitors and staff; redirect calls or provide routine information requiring a detailed knowledge of the organization’s functions, policies and procedures.
    2. Schedule and confirm medical appointments and receive and communicate messages for medical staff and
    3. Process incoming and outgoing faxes/
    4. Enter and format electronically based medical reports and correspondence and prepare spreadsheets and documents for
    5. Prepare correspondence, reports, statements, and other material as
    6. Interview patients in order to complete forms, documents and case histories (e.g. client registration).

 

  1. Office Coordination
    1. Coordinate and troubleshoot with property management and tradespeople to maintain smooth operation of the
    2. Oversees office security, such as the assignment of keys and alarm codes for approved personnel.
    3. Establish and maintain various internal office administrative support procedures such as document tracking and filing.
    4. Maintain inventory of office supplies and non-IT and non-clinical equipment (copier/fax, telephones, projectors, etc.); order supplies as required and arrange for servicing of office
    5. Assess administrative work flow and provide feedback and make recommendations to the Administrative Lead
  • Other Responsibilities
    1. Write, edit, proofread and finalize correspondence, reports, statements, forms, presentations, brochures, promotional materials and other documents as
    2. May train other staff in procedures and in use of current software and equipment
    3. Prepare draft agendas for meetings and take, transcribe and distribute minutes
    4. Coordinate meetings/events and assist with room arrangements and preparation of materials
    5. Participates in staff development, training and meetings as
    6. Willingness and ability to work flexible shifts, evenings, and
    7. Adhere to all policies and procedures of the Clinic including Occupational Health & Safety, IPAC, WHIMS, and PHIPA

Education and other Requirements:

  1. Completion of a one- or two-year college program or other specialized courses for medical secretaries.
  2. A satisfactory result of Vulnerable Sector Police Record Check is a requirement of this position.
  3. Valid Ontario G or G2 Drivers License and access to a reliable vehicle (preferred)

 

 

Experience:

  1. 1-2 years office clerical experience, medical office, community or hospital setting preferred
  2. Demonstrated ability to work effectively in a team
  3. Demonstrated ability to work in a multi-cultural setting

 

Knowledge:

  1. Electronic medical record systems (Telus PS Suites preferred), Internet, and Microsoft software
  2. Proficiency in the use of computers and various software applications

 

Skills:

  1. Excellent organizational
  2. Strong English written and verbal communication skills and plus a second language (Cantonese, Mandarin or Vietnamese)

Major Job Responsibilities:

  1. Clinical Function
    1. Provide clinical health assessments, diagnosis, nursing care and services to clients (individuals, families and groups) to promote a healthy lifestyle, to prevent and/or reduce incidence of disease and disability at the clinic and in the clients’
    2. Care for patients with both episodic and chronic illnesses and assist in addressing both medical and psychosocial needs.
    3. Work collaboratively in an interdisciplinary health team to provide comprehensive primary care to patients while demonstrating a high degree of emotional quotient, empathy and clinical expertise
    4. Develop, implement and evaluate medical
    5. Provide regular updates - reports regarding client care practices, trends, challenges and proposed
    6. Develop a plan of care based on patient needs, independently or in consultation with a collaborating physician and other members of the clinical

Position: Medical Clinic Receptionist (Summer Position)

Working Conditions:

Position: Social Worker, Full-Time Permanent position

 Working closely with the Lead Nurse Practitioner, interdisciplinary health professionals and the administrative team, the Social Worker will be responsible for the providing direct client care. The Social Worker provides counseling/therapy and other therapeutic functions for individuals, couples, families and the community. Working in collaboration with an interdisciplinary team, the Social Worker will support expertise and knowledge of health promotion, disease prevention, and chronic disease management across a client’s lifespan.

 

Major Responsibilities:

  1. Assessment, Treatment, Management and Planning
    1. In collaboration with Nurse Practitioners and other members of the interdisciplinary team, discuss and implement individualized care plan with the client/family care-giver based on best
    2. Provide psychosocial assessment, counseling, case management, psychotherapy and education/health promotion to individuals, couples, families, and
    3. Support nursing staff when necessary in the completion of preliminary mental health and addiction assessments, and work in collaboration with partner agencies to streamline referrals.
    4. Provide supportive counseling for clients with mild to moderate mental health
    5. Deliver short-term case management to individuals and
    6. Facilitate coordinated access to services in areas such as assistance with daily living, crisis intervention, treatment, health promotion and prevention through appropriate
    7. Evaluate achievement of client goals in the various areas, including but not limited to financial management, personal effectiveness, community integration, health and wellness, employment and community service, personal care and housing
    8. Collect and record in a timely manner client and service records in the Electronic Medical

Position: Nurse Practitioner (Relief), casual on-call basis

We are seeking a Nurse Practitioner (NP, Relief, casual on-call basis) who enjoys working with an inter-professional health team. The NP would be providing direct patient care in a community-based primary care clinic and provide home visits in the Scarborough area. The NP will report to the Lead Nurse Practitioner.

The ideal candidate will have advanced knowledge and skills in assessment, nursing, medical diagnosis, intervention, treatment management, drug therapy and referrals functions which are shared with collaborating physicians. Each NP will have a patient roster of approximately 800 patients and works with a team to provide family and client centered care.

 

Major Job Responsibilities:
